RN - First Connections Outreach (Full Time)

$5,000 sign-on bonus

We have an amazing opportunity to join our team as a Maternal Outreach Registered Nurse in our First Connections Program providing prenatal and postnatal short-term home visits. An outreach RN will plan and implement the assessment and coordination of care with women prenatally, newborns, and their families from intake through referral planning and transition. Coordination of care is provided in collaboration with families, birth hospitals, and other community referral sources. Meet with families in their homes to complete screening and assessment, determine appropriate referral plans, and document progress in referral engagement. Educate on various topics related to maternal-child health and infant development.

Qualifications:

• Licensed as a Registered Nurse by Rhode Island Department of Health
• Graduate from Board approved and accredited nursing program
• Three years of public health and community development experience
• Minimum of 1-3 years of direct service experience with pregnant mothers and/or families with young children
