GENERAL SUMMARY: Responsible for building strategic relationships, establishing parameters to build collaborative efforts and answering questions or concerns that come up for both parties. Provides many responsibilities, such as detecting collaborative opportunities, securing stakeholders buy-in for various collaborations and overseeing negotiations to help the organization receive maximum benefits. PRINCIPAL JOB FUNCTIONS: 1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values. 2. *Serves as a resource to the inter-disciplinary team and Care Management in meeting complex needs and barriers of patients and families throughout the process of discharge planning. 3. Helps to establish and maintain a regional network of relationships with community organizations and facilities. 4. Supports planning, development and implementation of pathways and programs. 5. *Maintains documentation and data on the progress related to community outreach. 6. *Increases community awareness through participation in outside events, professional groups, joining network events and local community involvement. 7. *Works closely with agency personnel to identify community resources. 8. *Develops and maintains a positive working relationship with facilities, local hospital systems and physician networks. Works with those groups to rapidly identify and address problems and develop best practices in support of patient care. 9. *Identifies barriers to patient transfers in the community and works with stakeholders to address challenges and improve access to care. 10. *Serves as a liaison with post-acute providers and the promotion of quality improvement across the continuum. 11. Collaborates with physicians, staff and post-acute providers. 12. *Compiles data, prepares reports and presents results. 13. *Defines opportunities for improvement through analysis of quality indicator reports and trends throughout the care continuum. 14. Coordinates appropriate monitoring and evaluation activities for post-acute care coordination programs and committees. 15. Assists with and facilitates education and collaboration in conjunction with Care Transitions leadership related to post-acute care coordination. 16. Maintains good working relationships with a variety of community service agencies. 17. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. 18. Participates in meetings, committees and department projects as assigned. 19. Performs other related projects and duties as assigned. (Essential Job functions are marked with an asterisk "*". Attach Addendum for positions with slightly different roles or work-specific differences as needed. REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: 1. Knowledge of strategies to mediate and manage expectations of post-acute continuum network. 2. Knowledge and clear understanding of assisted living, independent living, skilled facilities, home health care, acute rehab and long term acute care hospitals. 3. Knowledge of principles and practices of problem solving and barrier resolution methods. 4. Knowledge of computer hardware equipment and software applications relevant to work functions. 5. Ability to effectively listen and communicate effectively both verbally and in writing. 6. Ability to perform crucial conversations with desired outcomes. 7. Ability to establish and maintain effective working relationships with all levels of personnel, medical staff, volunteer and ancillary departments including past acute continuum network. 8. Ability to plan, organize and implement ongoing support for post-acute facilities and agencies to reduce barriers with transition planning. 9. Ability to maintain regular visits to post-acute continuum facilities and agencies. 10. Ability to problem solve and engage independent critical thinking skills. 11. Ability to maintain confidentiality relevant to sensitive information. 12. Ability to prioritize work demands and work with minimal supervision. 13. Ability to maintain regular and punctual attendance. EDUCATION AND EXPERIENCE: Bachelor's degree in business, healthcare administration or related field required. In lieu of a Bachelor's degree, eight (8) years of experience in long term care, marketing, community outreach, admissions or discharge planning required. Minimum of two (2) years of experience working in a healthcare facility required. Prior case management experience desired. OTHER CREDENTIALS / CERTIFICATIONS: Valid State of Nebraska driver's license required. Verification and maintenance of responsible driving record per facility standards required. PHYSICAL REQUIREMENTS: (Physical Requirements are based on federal criteria and assigned by Human Resources upon review of the Principal Job Functions.) (DOT) - Characterized as sedentary work requiring exertion up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body.
