GENERAL SUMMARY: Responsible for planning, development and oversight of the Structural Heart program. Facillitates care coordination of multiple complex patients to ensure positive outcomes and effective overall plan of care. Involved in clinical activities working closely with physicians and nursing staff. Provides patient/family education; participate in hospital and clinic care, while demonstrating knowledge and skill in addressing patient care needs of the complex patient. In addition to diagnosing and managing acute and chronic illnesses, emphasizes health promotion and disease prevention. Services include, but are not limited to: ordering, conducting and interpreting diagnostic and laboratory tests; prescription of pharmacologic agents and non-pharmacologic therapies. PRINCIPAL JOB FUNCTIONS: 1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values. 2. *Works closely with Cardiologists, Surgeons, clinic staff and hospital team to identify patients and provide care for the structural heart patient. 3. *Diagnoses patients by synthesizing and analyzing the collected data; formulates a differential diagnosis based on the history, physical examination, and diagnostic test results. 4. *Identifies and implements a patient treatment plan which may include ordering additional diagnostic tests; selecting appropriate pharmacologic and non-pharmacologic interventions; developing a patient education plan; making appropriate treatment referrals. 5. *Determines the effectiveness of patient treatment plans; documents patient care outcomes; reassesses and modifies treatment plans as necessary to achieve health and medical goals. 6. Participates as a team member in the provision of health and medical care; interacts and collaborates with professional colleagues to provide comprehensive care. 7. *Accurately documents patient findings, treatment plans and evaluations; documents interventions appropriately to ensure coding compliance for reimbursement purposes. 8. *Maintains patient confidentiality per departmental and facility protocols and procedures. 9. Presents patients at monthly TAVR meeting. 10. Monitors outcomes and works closely with the hospital clinical outcomes team to ensure accurate data for registry. 11. *Works closely with inpatient and clinic staff to provide clinical guidance and education to hospital and clinical staff. 12. Plans, directs, and provides care by implementing and developing protocols to assure the highest quality of care and service. Care shall be consistent with and/or exceed Joint Commission Standard and State and Federal regulations. 13. *Performs independently and with team members to meet the physical and psychosocial dimensions of the patients through collaboration with nursing staff, physicians, and other health care professionals. 14. Provides support, when requested, to the BH Inpatient APP service. 15. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. 16. Participates in meetings, committees and department projects as assigned. 17. Performs other related projects and duties as assigned. (Essential Job functions are marked with an asterisk "*". Attach Addendum for positions with slightly different roles or work-specific differences as needed. REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: 1. Maintains clinical competency as required for Nurse Practitioner or Physician Assistant licensure. 2. Knowledge of local, state and federal regulations related to nursing and medical services within practice area. 3. Knowledge of care of the critical and complex patient and their medications possible side effects on patients with co-morbidities. 4. Knowledge of process improvement and change management principles within the healthcare environment. 5. Knowledge of professional regulations and practices in giving and evaluating patient care. 6. Knowledge of computer hardware equipment and software applications relevant to work functions. 7. Knowledge of patient, family and staff education requirements. 8. Knowledge of medical clinic safety hazards and precautions. 9. Ability to incorporate diverse teaching strategies relating to documentation requirements to achieve quality management, clinical and operational goals. 10. Ability to facilitate change and manage projects through analysis, key end user engagement, and consensus building within an interdisciplinary environment. 11. Ability to perform physician exam of the complex patient. 12. Ability to evaluate the patient treatment needs by reviewing past medical/surgical history, performing physical exam and required follow up exams. 13. Ability to communicate effectively both verbally and in writing. 14. Ability to effectively organize and prioritize projects managing time working with minimal supervision. 15. Ability to assess, plan, implement and evaluate the clinical theoretical processes as they relate to patient care. 16. Ability to problem solve and engage independent critical thinking skills. 17. Ability to work independently, take direction and work collaboratively with other health team members from diverse backgrounds. 18. Ability to develop effective working relationships with all co-workers, clinical and non-clinical colleagues. 19. Ability to maintain regular and punctual attendance. EDUCATION AND EXPERIENCE: Master of Science degree in Nursing with current Advance Practice Registered Nurse licensure from the State of Nebraska required. Certification as an Adult Nurse Practitioner, Family Nurse Practitioner or other related specialty area required, or, Master of Science in Physician Assistant & certification through AAPA and current State of Nebraska license. Certification with the U.S. Drug Enforcement Administration for prescriptive authority required. Minimum of three (3) years clinical practice experience required. |
