Description YMCA International Services ("the Y"), as a branch of YMCA of Greater Houston, works to meet the needs of Houston's refugee and immigrant communities. The Y provides holistic services that focus on clients' integration and self-sufficiency. The Intake Case Manager will be responsible for completing the initial assessment and linking clients to internal programs and/or other appropriate community resources. Duties /Responsibilities Responsibilities of this position include, but are not limited to, the following:
- Schedules initial assessments based on priority level and urgency
- Conducts eligibility screening and determines eligibility for programs
- Uses strong assessment skills to conduct comprehensive assessment of clients' physical and emotional needs
- Links clients to in-house services and programs to meet goals identified on the service plan
- Maintains all required documents and assessments in clients' files as required
- Maintains confidential information and exercises sound judgment and discretion
- Maintain regular communication with Program directors to keep abreast of services offered
- Complete airport pickup as assigned
- Perform other duties as assigned
- Report suspicious and inappropriate behaviors and policy violations.
- Follow mandated abuse and incident reporting requirements.
- Meet timelines and deadlines related to supporting systems. Example includes but are not limited to Kronos Workforce Ready for review and approval of time sheets.
- Other duties, as assigned.
Annual Outcomes and Indicators Depending on yearly performance measures, the Intake Case Managers are directly responsible for:
- Complete initial needs assessment and direct eligible clients to appropriate programs.
- Be able to process 35 clients per week.
- Conduct a follow up to assure client's needs are met.
- Maintain complete and accurate documentation in the case file.
Requirements
- Bachelor's Degree in Social Work, Psychology, or related field
- Strong assessment skills and strong writing skills
- Excellent organizational and strong time management skills
- Strong interpersonal and communication skills; experience working with Limited English Proficiency population preferred
- Ability to work both independently and with a team
- Ability to work with diverse populations
- Comfortable working in a fast-paced environment
- Must be able to transition from one task to another based on demand for services
- Strong proficiency with Microsoft Excel, Outlook and Word required.
- Bilingual in English/Spanish is required
- Texas Driver's License, clean driving record, own vehicle, and applicable auto insurance
